Immigration Exams

Overview and FAQ

Dr. Apeksha Desai, MD is a licensed Civil Surgeon and can perform Immigration Medical Exams. These exams are required to obtain a green card with USCIS or other immigration benefits.

Information about the exam

  • You will be given an appointment date and time with the doctor.
  • Please arrive 15 min before your appointment time. The office staff will check you in, and you will be asked to complete screening and medical history forms.
  • The health care provider will review your medical history, vaccination records and perform a standard physical exam.
  • Once we receive the results of your tests, we will schedule a follow up to review the test results, recommend additional vaccines if indicated and to complete, date and sign the form (additional cost noted in Fees, payment methods, etc.).

Please note: Per USCIS regulations, we cannot accept blood or urine tests ordered by another provider. We have to order these tests and receive the results directly from the lab. Following the above guidelines will reduce your total out of pocket costs.

Submitting the results of your immigration exam

After your exam is concluded, you will receive the completed Form I-693 with supporting documentation in a sealed envelope. Do not open or break the seal on this envelope. The sealed envelope must be submitted with your application to USCIS for processing. A copy of the completed Form I-693 and lab results will also be given to you for your records.

Frequently asked questions

Why do I have to pay for the second visit when I am just collecting the I-693 form?

We reserve a 15-minute time slot for the doctor to meet with you. During this appointment she will review additional paperwork including pending vaccine records. She will explain your test results and complete your I-693. You will sign the form in her presence during this appointment (as required by Immigration Law).

Are there any "No-Show" fees?

  • Yes. If you do not come for your appointment there is $50 "No-Show Fee".
  • We also charge a $50 rescheduling fee if you reschedule your appointment within 24 hours.

Fees, payment methods, etc.

Exam fees

  • Initial visit: $249.
  • Subsequent visit: $149.
  • In most cases we should be able to give you the completed form during the second visit.
  • If your paperwork is incomplete during the second visit, you will be required to return for subsequent visits incurring $149 fees for each visit.

Other typical costs

Lab testing often totals about $140 to $250. Vaccines, when needed, may total about $70 to $700. Interpretation services are $50 when we use our interpretation service.

You can call your insurance company to ask whether labs or vaccines will be covered. ICD-10 codes used include Z11.7, Z11.3, and Z23. If your insurance company asks for CPT codes, contact the lab (LabCorp, CPL, or Quest). Required labs and vaccines are listed under Labs, tests, what to bring along.

Payment method

We accept cash, credit and debit card. We do not accept personal checks.

Please Note: Fees are non-refundable. You are paying for the Civil Surgeon's time to perform the exam and complete paperwork.

Labs, tests, what to bring along

Required laboratory tests

  • QuantiFERON TB (required).
  • RPR for Syphilis (required).
  • Neisseria gonorrhea urine test (required).
  • MMR Titer (not required if you have proof of 2 doses of MMR vaccine, or lab result showing titer values).
  • Varicella Titer (not required if you have proof of Varicella vaccine or lab result showing titer values).
  • Hepatitis B Titer (not required if you have proof of 3 doses of Hepatitis B vaccines, or lab results showing titer values, or over 59 years of age).

Vaccine requirements for adults (18 and above)

  • If you have insurance, you can get the vaccines at your PCP's office or at a pharmacy.
  • The following vaccines are required for adults (18 and above):
  • COVID-19 Vaccine
  • Tdap or Td
  • Influenza Vaccine (from Oct 1 to March 31)
  • Varicella Vaccine or varicella titer results
  • MMR Vaccine or MMR titer results (Not required for 65 years and older)
  • Pneumonia Vaccine (Required for 65 years and older)
  • Hepatitis B Vaccine or Hepatitis B titer results (Not required for age 60 and older)

Interpretation services

If you don't speak English and we use our "interpretation service", there is an additional cost (see Fees, payment methods, etc.). You are welcome to bring someone you trust to help interpret for you to save on this cost.

What to bring during your exam

A valid passport or Federal government-issued photo ID

Please note that if your Driver's License says "Not for Federal Identification" we cannot use that as identification document for I-693.

Proof of vaccines

The following information is required if vaccines were administered in USA:

  • Name of Vaccine
  • Date of Administration
  • Manufacturer
  • NDC Code
  • Lot Number
  • Expiration Date
  • If vaccines were given outside USA, then either the original, or official documentation is required as proof.
  • If vaccine records are not in English, you will be required to bring official translation along with the original non-English documents.

List of any medications that you take regularly

If you have been treated for tuberculosis (TB), syphilis, or gonorrhea

A written certificate describing medications taken, start and stop dates of medications and lab/imaging results is required.

If you have been treated for mental illness, alcohol or drug abuse

A written certificate indicating diagnosis, treatment, and prognosis is required.
